Every 300 km or after riding in rain or mud, with degreaser and a small brush. Check the teeth after around 3,000 km of hard use or if you hear noise when shifting.
Yes, it's the same part: the two small toothed wheels in the rear derailleur cage. The upper one guides the chain onto the cassette and the lower one keeps it under tension.
